UCCJEA Declaration.  Declaration Under Uniform Child Custody Jurisdiction and Enforcement Act Must be completed using Form FL-105/GC-120) (Fam. Code, § 3429, 3402(d).)

Proof of Personal Service.  Proof of timely personal service of the notice of hearing and a copy of the petition (Prob. Code, § 1511) on parents, OR Consent to Appointment of Guardian and Waiver of Notice (Judicial Council form GC-211) OR adequately supported request to dispense with notice (CRC 7.52; see Local Court form SC-6014).

Proof of Timely Mailed Service.  Proof of timely mailed service of the notice of hearing and a copy of the petition (Prob. Code, § 1511) on paternal and maternal grandparents, and siblings, OR Consent to Appointment of Guardian and Waiver of Notice (Judicial Council form GC-211) OR adequately supported request to dispense with notice (CRC 7.52; see Local Court form SC-6014).

PLEASE NOTE: The request to dispense with notice is not supported by any evidence. A proper Request to Dispense with Notice should contain facts supporting a finding of due diligence to locate (Prob. Code, § 1511(g)(1), CRC 7.52; Local Court Form SC-6014) or that notice is contrary to the interest of justice.  (Prob. Code, § 1511(g)(2).)

Proposed Letters (Form GC- 250)  Proposed Letters are considered the same as proposed orders and must be submitted in compliance with Local Rule 1724.
