Appearances required. The following must be submitted:

Proof of Service.  At least 15 days before the hearing on a petition for modification or revocation, notice of the time and place of the hearing shall be given on Judicial Council form GC-020 and timely mailing notice of the petition to the (1) conservator; (2) conservatee; (3) spouse of the conservatee or the domestic partner of the conservatee; and to (4) any person who has requested special notice of the matter, as provided in Section 2700. (Prob. Code, §§2351(c); 1460.)

Proposed Order Appointing Conservator (GC-340).  A proposed order that contains the proposed modification of powers must be submitted. “Proposed orders or judgments shall be submitted on or before noon at least five court days prior to the hearing. Failure to submit a timely proposed order may delay the hearing or entry of the order.”  (Local Rule 1724.)  The proposed order submitted does not contain any different orders than the original order issued by this court.

Proposed Letters of Conservatorship (GC-350).  A proposed Letters that contains the proposed modification of powers must be submitted. “Proposed orders or judgments shall be submitted on or before noon at least five court days prior to the hearing. Failure to submit a timely proposed order may delay the hearing or entry of the order.”  (Local Rule 1724.)

The proposed conservatee is not expected to attend the hearing.  (Prob. Code, § 1825.) (Prob. Code, § 1825(a)(2)[medical inability].)
