Confidential Supplement to Duties and Liabilities of Personal Representative (Form DE-147(S)).  Petitioner failed to properly fill out the DE-147, which warrants the filing of an amended Duties and Liabilities form and the requirement of the Confidential Supplement to Duties and Liabilities of Personal Representative. The proposed personal representative must fill out and sign this form and submit it to the court. (Prob. Code, § 8404.) Please amend the Duties and Liabilities of Personal Representative (DE-147) and include the Confidential Supplement to Duties and Liabilities of Personal Representative (DE-147(S)).
