Conservatee's Care Plan (Form GC-355/GC-356) The conservator shall file with the clerk of the court a care plan for the care, custody, and control of the conservatee A copy of the care plan must be served on the Conservatee and the Conservatee’s attorney. (Prob. Code, § 2351.2)
