This matter was set for hearing upon appointment of the conservator to ascertain whether the conservator has complied with certain obligations, and it is noted that statutory compliance has not been achieved.

The following must be submitted:

Conservatee's Care Plan (Form GC-355/GC-356) Prob. Code, § 2351.2 was amended on January 1, 2024. Effective January 1, 2025, the new Judicial Council forms were produced and are mandatory. In this case, an older version of the form was used that is not in compliance with the current requirements.

This matter is already set for further hearing on 01/14/2027 to review the court investigator’s 6-month report.
