UCCJEA Declaration showing at least 5 years history.  Declaration Under Uniform Child Custody Jurisdiction and Enforcement Act Must be completed using Form FL-105/GC-120) (Fam. Code, § 3429, 3402(d) [“child custody proceeding” includes guardianships].)  The current declaration does not show that Minor has lived in California for last 6 months and is a resident of or temporarily resides in Santa Barbara County. (Prob. Code, § 2200-2202.)
