The following must be submitted before petition may be approved:

Supplement re: Full Recovery.  Adequate documentation that the minor has fully recovered, or is expected to fully recover from his injuries.  There were no attachments supporting the allegation at paragraph 9 of the petition.  Current medical records must support a finding that the minor will fully recover or give a long-term prognosis the court can rely upon to determine whether the settlement is sufficient to provide for the medical needs of the minor.
