Appearances required.  The following must be submitted:

Proposed Order Establishing Order of Right to Dispose of Remains. A proposed order must be submitted that contains the proposed ruling prayed for in the petition, and contains an order organizing the rights to dispose of remains in the proposed hierarchy outlined in the petition.  The proposed order must also include the proper findings and rulings the court must make in order to grant the prayer for relief.
