Proof of Personal Service. There is no Proof of Service to any parent of the proposed wards on file, nor adequately supported request to dispense with notice.  The Declarations of Due Diligence filed on November 13, 2025, do not show sufficient effort to locate the parents.

Proof of timely personal service of the notice of hearing and a copy of the petition (Prob. Code, § 1511) on all living parents, OR Consent to Appointment of Guardian and Waiver of Notice (Judicial Council form GC-211) OR adequately supported request to dispense with notice (CRC 7.52; see Local Court form SC-6014).

Proof of Timely Mailed Service to Relatives.  Proof of timely mailed service of the notice of hearing and a copy of the petition must be filed showing service (Prob. Code, § 1511) on all grandparents, OR Consent to Appointment of Guardian and Waiver of Notice (Judicial Council form GC-211) OR adequately supported request to dispense with notice (CRC 7.52; see Local Court form SC-6014).
