The following must be submitted:

Declaration of Third Party. In order for the court to rely upon the allegations in the application, the applicant must 1) show how the facts stated on the certificate of birth are in conflict with evidence related to that fact, and 2) file a declaration under the penalty of perjury from at least one other “credible person” with personal knowledge of the conflicting facts. (Ibid.) The application must only ask for relief related to “parentage on the originally registered certificate of birth.” (Cal. Hlth. & Saf. Code, § 103230.)
