The following issues are outstanding for the Petition for General Guardianship, and must be corrected:

Adequate ICWA Inquiry.  Inquiry of paternal parent is required, because inquiry on both sides of the minor’s family is required and the inquiry has not been done on the paternal side. (CRC, Rule 5.481(a).)  The parents of the child must, if possible, complete the Parental Notification of Indian Status (form ICWA-020). (Welf. & Inst. Code, § 224.2; Prob. Code, § 1459.5(b); Cal Rules of Court, rules 5.481(a)(1), 7.1015(c).)
