Supplements/corrections submitted after Monday, October 16, 2023, at 5pm will not be considered for the October 19, 2023, hearing (Local Rule 1721(c)(2)(2)). If deficiencies cannot be cured by that date, a continuance must be secured pursuant to Local Rule 1722(a)(1). Deficiencies must be corrected, or a continuance secured prior to hearing, or an appearance is required.

This matter was previously continued to provide the Conservator additional time to comply with certain obligations and for filing of the Conservator’s 1st account and report.

Compliance Hearing. With regards to the compliance hearing, it is noted that statutory compliance has not been achieved and the items below must be submitted.

Notice of Conservatee’s Rights (Form GC-341) (Prob. Code, § 1830(c)). This form must be timely mailed to the conservatee and to the conservatee’s relatives within the second degree (grandparents, parents, siblings, adult children, and adult grandchildren)

Determination of Conservatee's Appropriate Level of Care (Form GC-355) (Prob. Code, § 2352.5)

Inventory & Appraisal (Form GC-040) (Prob. Code, § 2610)

Proof of Service – Notice of How to File Objection to I & A (GC-042) (Prob. Code, § 2610) Proof of Service mailed to the conservatee, the conservatee’s spouse, and the conservatee’s parents and adult children

Proof of Recording Letters of Conservatorship (Prob. Code, § 2313). If the conservatee owns property, proof of recording the Letters of Conservatorship must be filed for each county in which conservatee owns property.

Accounting. With regards to the 1st account and report, it is noted it has not been filed
