Supplement to the petition.  A supplement is required to give the court more facts as to the following issues that are not clear from the scant allegations in the petition:

1. The extent of the petitioner’s rehabilitation from the circumstances relied upon by this court to find the petitioner unfit when the guardianship was granted.
2. The extent to which the petitioner established a stable living situation.
3. The extent to which the petitioner has stayed involved in the child's life so the child's emotional connection to the parent is strong enough to warrant having the child leave the guardian's home and return to living with the parent.
4. The extent to which a custody determination re: the wards’ father should be made if guardianship is terminated. This necessarily includes the extent the wards’ father has made the above progress as well.
