Fee Waiver

Ask for a Fee Waiver if you can't afford filing fees

These are the minimum forms required by the court for the typical or standard fee waiver application. These forms are free and can be obtained from the court's records clerk, or you may fill them in below and print to your own printer. Your case may require additional forms. Should you need any additional forms, the LRC staff can assist you with finding more information.
